| Reimbursement / Fund Request Form | Used to request reimbursement from the SLES PTA, or to ask the PTA to pay an expense directly; please submit the Pre-Approval Form before incurring the expense |
| Funds Collected Summary Form | Used to verify amounts received; submit to SLES PTA Treasurer along with actual funds received |
